Flying Fish Swimming School Membership Agreement

 

Registration Fee
Registration fee $25 is due at registration for new students, $15 for the 2nd sibling.$15 will be charge if you want to switch class time after class registered.

 

Policy

 

  • Switching of classes or times is not permitted for all levels.
  • No make up for any missed class/training for all level
  • No credit is given for any missed class/training

 

I understand that no make up classes are offered for all levels unless lessons are cancelled by FFSS due to unforeseen problem.In the event that FFSS cancels, a make up lesson may be held on an alternate date, time, and /or location.If FFSS is unable to offer a make-up, a credit will be issued.

I understand that coaches are hired on a pre-registration basis and no refunds will be given after 1st lesson/class.I understand that if I cancel class 1-3 days prior to the start of session.I will be charged 20% of the total fees.

 

I understand that for the safety of all, I may watch lessons from the designated parent viewing area only.As a parent that I understand children at all age must be accompanied and closely supervised by a responsible adult (18 years or older) while in the Pool area.Flying Fish Swimming School is not responsible for safety of any siblings, Children who are not attending the class at the facility.

Waiver/Release of liability


By sending the form online, I certify swimmers are in good health to swim, and have no physical condition that would prevent participation in this activity.I give permissions any representative, or coaches of Flying Fish Swim School to have the participant treated in any medical emergency if accident of injury occurs. Further, the participant and /or parent/guardian agree to pay all the cost associated with medical care and transportation for the participant.

 

We understand and assume all incidental risks involving in swimming.I recognize that there are risks in the sports of swimming, including but not limited to paralyzing injuries and death.I agree, on behalf of myself and my child, our heirs, personal representatives and assignees, not to make any claim against or sue the City of Fremont, Ohlone College, The City of Pleasanton, City of Sunnyvale, Sunnyvale Middle School, NASA Exchanges, City of Newark, Flying Fish Swimming School, also I agrees to hold harmless and waive all claims against the pool facility owner, management organization, Flying Fish Swimming School, their officers and employees.

 

 

 

 

 

Hold Harmless Agreement

 

Each participant (and if under age 18, his or her parent of guardian) in the Flying Fish Swimming school (i) is aware of the specific dangers and risks of injury inherent in a program of competitive swimming and training, (ii) hereby assumes all such risks, and (iii) hereby agrees to hold harmless from, and to release from, and to waive, all claims of liability for injuries or damage against either Flying Fish or the facilities it uses or any of employees of Flying Fish. Each participant (and if under 18, his or her parent of guardian) specifically agrees to hold harmless from, and to release from, and to waive, and claims against either Flying Fish Swimming School based on lack of suitability of such participant for the Flying Fish swim program; any decision to enroll or continue in such program is exclusively that of the participant and his or her family.

 

Sanctions: Flying Fish Swim School maintains the right to terminate any membership at any time with cause in the interest of our vision, mission, objectives, and safety issue.

 

I Further understand that Flying fish Swimming School reserves the right to photograph and film facilities, activities, and program participants for potential use in our marketing efforts, which may include, but not limited to, brochures, flyers, TV commercials, and theFlying Fish Swimming School's website.All marketing materials will remain the property of the Flying Fish Swimming School.
